#1. Human Factor Considerations & Health Information Technology (International Ballroom West) This breakout session will bring to light a rich conversation on the importance of establishing a motivation and goal for how you engage consumers with information via technology. Simply giving consumers online access to their data alone is not enough to strike engagement or influence behavior change and health outcomes. Making sure consumers and end users can find the information they need and understand is critical. What are some of the human factors considerations that must be employed with enabling consumers facing technologies? How do you sustain engagement with the technologies? #2. The Value of Person-Centered Care Planning Through Health Information Technology (Jay Room) Care Plans offer a valuable platform to support care coordination and enhance individual engagement. Stakeholders across the spectrum of care have increasingly looked to the concept of a comprehensive, dynamic, shared care plan as a vehicle for resolving communication issues and achieving highly coordinated, person-centered care. The availability of new and emerging technologies to support the creation, exchange, and use of these dynamic care plans provides unique opportunities to empower individuals. This session will highlight individual priorities for IT-enabled, person-centered care planning and user stories and discuss how stakeholders can use Care Plan 2.0 as a roadmap for engaging individuals, families, and providers in achieving the best possible health while keeping the person at the center. Breaking through the Noise: Using Data to Make More Informed Decisions (Oak Lawn Room) Health care consumers are faced with a series of complex choices when it comes to managing their health and health care. What plan works best for me? Which doctor should I choose? Which treatment is best? Do I really need to have that test done? There are many tools that exist to help consumers make more informed decisions about their health and care based on cost, quality, and effectiveness. This breakout session will explore how to get consumers and providers to use cost, quality, and effectiveness data to make more informed decisions and to use best practices for increasing consumer awareness and use of these tools. Health IT: Improving Access to Care and Empowering Individuals and Caregivers Across the Care Continuum (Piscataway Room) This session will focus on ways to advance knowledge about health-IT enabled methods and tools that empower individuals, particularly older adults and people with disabilities and their caregivers, to engage in their daily health management from community-based settings according their individual goals and preferences in convenient and meaningful ways to improve their individual health outcomes and overall well-being.
